At your first visit to Bear Town Chiropractic Clinic staff will take a complete medical history and Dr. Russ will perform a comprehensive physical examination. If necessary, X-rays will be taken within our facility to help determine the cause of your ailment. Upon review of your medical history and completion of your physical examination, if Dr. Russ determines that you could benefit from chiropractic treatment he will provide you with a treatment plan.

The first visit usually takes about 45 minutes.

WHAT TO BRING

Whether it is a new injury or a chronic pain you have dealt with for years, please bring all pertinent medical records, a list of any medications you are currently taking, recent X-rays or scans and your insurance information.

To simplify your first visit you can print off the Bear Town Chiropractic health history form found below, complete it prior to your appointment and then bring it to your first visit with Dr. Russ.
